Bring the following items with you when you
come to the registration appointment:
The child's original Birth Certificate
The child's Immunization Record
The child's Social Security Number
Custody Papers (if applicable)
The name and address of the former school the child attended
Proof of residency for the parent/guardian as listed below
If you are renting:
You must bring a copy of the Residential
Occupancy Permit. (You can obtain this from your landlord or
Code Enforcement at City Hall, 233-2127.)
If you own a home:
You must bring a DEED or other indication of
Real Property Ownership, such as a property tax bill or mortgage
statement, ALONG WITH ONE OF THE FOLLOWING:
Current Electric Bill
Current Phone Bill
Current Drivers License
Current record of Voter Registration
Custody/guardianship
A legal notarized document must be provided
specifying student custodial agreement
Parents/guardians should also complete the
Registration Form, the
Student Information Card, the
School Health Exam Form and the
Residency Questionnaire.
