Sports physicals

The following dates will be offered to all middle school and high school students wishing to participate in a fall sport.

Sports physicals will be offered on:

Tuesday, August 5 - 9 a.m. – 11:30 a.m.

Wednesday, August 6 - 9 a.m. - 11:30 a.m.

Students must return updated completed forms by their parent in order to receive participation approval.

On Monday, Aug. 4, from 8 a.m. - 12 p.m. all middle school and high school students who wish to participate in a fall sport and had physical done by their own physician must return proof of their recent physical and the update form. The update form will be distributed by the coach prior to this date and is to be completed by parent.
